ADPKD fitness guide: CKD stage-specific exercise plans, aerobic and strength training guidance, heart rate intensity calculation, exercise contraindications, cyst bleeding prevention, and daily activity recommendations.

All exercise plans must be assessed by a doctor before implementation, especially for mid-to-late stage patients, those with a history of cyst bleeding, those with greatly enlarged kidneys, or those with cardiovascular disease. If you experience severe flank or abdominal pain, gross hematuria, chest tightness, or chest pain during exercise, stop immediately and seek medical care.
Why Exercise Is Important for ADPKD Patients
Regular exercise is an important part of comprehensive ADPKD management that patients can actively control. Compared to the general population, ADPKD patients face higher cardiovascular event risk, are more prone to hypertension and obesity, and face the challenge of muscle loss during disease progression and dialysis preparation. Scientific exercise can provide benefits in the following areas, though specific effects vary by individual and require medical assessment.
- Blood pressure control: Regular aerobic exercise can lower resting blood pressure by approximately 5β7 mmHg, serving as a natural complement to RAAS inhibition (such as ACEI/ARB therapy). Blood pressure control is a core goal in slowing ADPKD progression, and exercise works synergistically with sodium restriction and medication.
- Cardiovascular health: ADPKD patients have elevated risk of left ventricular hypertrophy, arrhythmia, and coronary artery disease. Moderate-to-low intensity aerobic exercise can improve endothelial function and lower resting heart rate, helping reduce cardiovascular event rates.
- Weight control: Obesity accelerates kidney function decline and makes hypertension harder to manage. Exercise combined with diet management helps maintain a healthy weight (BMI 18.5β23.9 is a general reference range; individual targets set by your doctor).
- Mental well-being: Anxiety and depression are common in chronic disease patients. Regular exercise promotes endorphin release and improves sleep quality, serving as an adjunct to psychological support but not replacing necessary psychological or pharmacological treatment.
- Muscle mass maintenance: CKD patients are prone to protein-energy wasting (PEW) and sarcopenia. Building muscle reserves through strength training in advance helps prepare for possible future dialysis and reduces fall and infection risk.
Tip
The benefits of exercise come from long-term accumulation. A single bout of intense exercise does not provide extra benefits and may instead increase the risk of cyst bleeding or cardiovascular events. Gradual progression and long-term consistency are more important than intensity.
Exercise Intensity and Heart Rate Guidance
Grasping the appropriate exercise intensity is key to safe exercise. Too low an intensity has limited effect, while too high can bring risk. The following three methods can help ADPKD patients assess and control exercise intensity β using them in combination is recommended.
Target Heart Rate Calculation
Target heart rate is the most commonly used objective intensity indicator. The calculation method below applies to patients without arrhythmia who are not taking heart rate-affecting medications (such as beta blockers); those taking such medications should rely primarily on the Rating of Perceived Exertion and the talk test.
- Maximum heart rate = 220 β age (beats per minute)
- Moderate intensity = Maximum heart rate Γ 60%β70%
- Low intensity = Maximum heart rate Γ 50%β60%
Example: A 40-year-old patient, maximum heart rate = 220 β 40 = 180 bpm; moderate intensity target heart rate = 180 Γ 60%β70% = 108β126 bpm. Maintaining heart rate in this range during exercise constitutes moderate intensity. ADPKD patients are generally advised to exercise primarily at moderate intensity and avoid prolonged high-intensity exercise.
Rating of Perceived Exertion (RPE)
The Rating of Perceived Exertion (RPE) requires no equipment and is suitable for all patients, especially those taking heart rate-affecting medications. A common 1β10 scale:
- 1β2: Very easy (e.g., slow walking) β warm-up or recovery
- 3β4: Easy to somewhat tired (e.g., brisk walking) β low intensity
- 5β6: Somewhat tired to tired (e.g., jogging, swimming) β moderate intensity, recommended range for ADPKD patients
- 7β8: Tired to very tired β high intensity, ADPKD patients should avoid
- 9β10: Very tired to maximum β strictly prohibited
Talk Test
The talk test is the simplest on-the-spot intensity assessment method, requiring no equipment:
- Moderate intensity: Can talk but cannot sing β breathing is faster but you can still hold a full conversation. This is the recommended intensity for ADPKD patients.
- Low intensity: Can talk and sing β breathing is basically steady, suitable for warm-up, cool-down, or late-stage patients.
- High intensity: Difficulty talking, can only say short phrases β breathing is rapid. ADPKD patients should avoid this intensity.
Aerobic Exercise Plans
Aerobic exercise is the foundation of exercise for ADPKD patients, primarily improving cardiopulmonary function, assisting with blood pressure reduction, and controlling weight. The following plans apply to general situations; specific implementation requires medical assessment and adjustment based on individual kidney function stage, kidney size, and comorbidities.
Walking / Brisk Walking
- Suitable for: All CKD stages β the safest and most accessible aerobic exercise
- Plan: 5 times per week, 30β45 minutes each, moderate intensity (RPE 4β6)
- Progression: Start with 15 minutes per session, add 5 minutes per week, gradually reaching target duration
- Caution: Choose flat surfaces, wear well-fitting athletic shoes; late-stage patients can split sessions (e.g., 15 minutes morning and evening)
Swimming
- Suitable for: G1βG3 stage, joint-friendly, suitable for overweight patients or those with knee discomfort
- Plan: 3 times per week, 30 minutes each, freestyle or breaststroke
- Caution: Avoid diving and vigorous kicking; water temperature should not be too low (recommended 26β30Β°C) to avoid cold-induced blood pressure fluctuations; G4βG5 stage requires medical assessment before deciding suitability
Cycling
- Suitable for: Indoor stationary bikes or outdoor flat-road riding, suitable for G1βG3 stage
- Plan: 3β4 times per week, 30β40 minutes each, moderate intensity
- Caution: Avoid high-intensity interval training (HIIT); for outdoor riding, choose flat roads and avoid heavy traffic; adjust seat height appropriately to avoid excessive forward lean that increases intra-abdominal pressure
Tai Chi / Baduanjin
- Suitable for: All stages, especially recommended for late-stage and elderly patients
- Plan: 20β30 minutes daily, can be split into morning and evening sessions
- Benefits: Combines balance training (fall prevention) with mind-body relaxation. Movements are gentle with minimal intra-abdominal pressure changes, making them kidney-safe. Baduanjin movements such as "Single Lift to Regulate Spleen and Stomach" and "Two Hands Hold the Sky to Regulate the Triple Burner" are gentle stretches suitable for daily practice
Yoga
- Suitable for: G1βG3 stage, focusing on flexibility and breath control
- Caution: Avoid abdominal twisting and inversion poses (such as headstand, shoulder stand), and avoid deep forward folds that compress the abdomen
- Recommended poses: Mountain pose, tree pose, cat-cow, child's pose, supine relaxation pose β these are gentle, do not increase intra-abdominal pressure, and are suitable for ADPKD patients
Tip
Regardless of which aerobic exercise you choose, include 5β10 minutes of warm-up and 5 minutes of cool-down. Exercise intensity and duration must be adjusted by your doctor based on your individual situation β do not blindly pursue exercise volume.
Strength Training
Strength training (resistance training) is equally important for ADPKD patients, especially for maintaining muscle mass and bone density. However, strength training requires extra care to avoid sharp increases in intra-abdominal pressure that could stress enlarged kidneys. The following plans require assessment by a doctor and rehabilitation specialist before implementation.
Why Strength Training Is Needed
- Maintain muscle mass, prevent sarcopenia: CKD patients lose muscle faster than the general population. Building muscle reserves through strength training in advance helps prepare for future dialysis and reduces fall and infection risk.
- Maintain bone density: CKD patients have elevated osteoporosis and fracture risk. Moderate weight-bearing training can stimulate bone formation and maintain bone density, working synergistically with vitamin D and calcium management.
- Improve metabolism: Increased muscle mass helps raise basal metabolic rate, assisting with blood sugar and weight control.
Safe Strength Training Plan (G1βG3 Stage)
The following plan applies to patients with G1βG3 kidney function, no history of cyst bleeding, and stable blood pressure control. Requires medical assessment before implementation.
- Resistance band training: 2 times per week, 15β20 minutes each, using light-to-moderate resistance bands for upper body pulls, leg abductions, seated rows, etc. 12β15 repetitions per set, 2β3 sets
- Bodyweight training: Squats (half squats, thighs at approximately 45Β° to the floor, no need to squat to parallel), wall push-ups (push-ups against a wall to reduce difficulty and intra-abdominal pressure), glute bridges β 12β15 repetitions per set, 2β3 sets
- Light dumbbells: 1β2kg dumbbells for bicep curls, lateral raises, shoulder presses, etc. 12β15 repetitions per set, 2β3 sets; movements should be slow and controlled, avoiding explosive force
Strength Training Contraindications
β Strength Training Contraindications
The following movements may sharply increase intra-abdominal pressure or blood pressure, posing risk to enlarged kidneys. ADPKD patients should avoid them:
- Avoid heavy weights: Unilateral loads > 5kg should be avoided, especially for movements involving core exertion
- Avoid Valsalva maneuvers: Holding your breath while exerting causes blood pressure and intra-abdominal pressure to spike instantly. Always maintain natural breathing during training β exhale on exertion, inhale on relaxation
- Avoid movements that sharply increase intra-abdominal pressure: Deadlifts, heavy squats, heavy bench presses, weighted sit-ups, etc. should be avoided
G4βG5 Stage Strength Training
- Light activity only: Light resistance band training, range-of-motion exercises, flexibility stretching
- Requires doctor and rehabilitation specialist assessment: G4βG5 patients have reduced stamina and higher risk of electrolyte imbalance. Any strength training must be done under professional guidance
- Focus: Maintaining joint flexibility and basic muscle strength, preventing bed-rest complications, rather than building muscle
CKD Stage-Specific Exercise Plans
As kidney function declines, exercise plans need corresponding adjustment. The following stage recommendations are general references; specific plans require medical assessment based on individual circumstances.
G1βG2 Stage
- Aerobic exercise: 150 minutes per week at moderate intensity (can be divided into 5 sessions of 30 minutes each)
- Strength training: 2 sessions per week of light training (resistance bands or bodyweight)
- Can participate in: Swimming, cycling, tai chi, yoga, brisk walking, low-intensity aerobics
- Caution: Even with early-stage normal kidney function, avoid contact sports and high intra-abdominal pressure movements
G3 Stage
- Aerobic exercise: 120β150 minutes per week, slightly reduced intensity (primarily RPE 4β5)
- Strength training: Primarily resistance bands, avoid weights, 2 sessions per week
- Caution: Monitor blood pressure before and after exercise, avoid large blood pressure fluctuations; watch electrolyte levels β patients with hyperkalemia should avoid strenuous exercise
G4βG5 Stage
- Aerobic exercise: 60β90 minutes per week, primarily low intensity (RPE 3β4)
- Strength training: Only range-of-motion and flexibility training, requires medical assessment
- Recommended: Walking, tai chi, Baduanjin β gentle, safe, and adjustable at any time
- Caution: Fatigue is common; can split into multiple short sessions (e.g., 10β15 minutes each); avoid exercising alone over long distances
Dialysis Stage
- Dialysis days: Light activity during non-dialysis hours; avoid strenuous exercise on dialysis days
- Recommended: Intradialytic exercise (e.g., cycling during dialysis, if facilities allow) β studies show it can improve dialysis adequacy and muscle function
- Caution: Avoid weight-bearing or blood pressure measurement on the arm with the arteriovenous fistula; protect the fistula-side limb; monitor blood pressure and weight changes before and after exercise
Tip
Stage-based plans are only reference frameworks. Patients at the same stage may have very different fitness levels. Please follow your doctor's assessment and your own feelings β do not mechanically apply the numbers.
Exercise Contraindications and Risks
ADPKD patients' kidneys are often enlarged; external impact may cause traumatic cyst rupture and bleeding. Sharp increases in blood pressure and intra-abdominal pressure from strenuous exercise also pose risks. The following contraindications must be strictly observed.
Absolute Contraindications (Avoid)
β Absolutely Prohibited Exercises
The following exercises should be avoided by all ADPKD patients, regardless of kidney function stage:
- Contact sports: Soccer, basketball, rugby, ice hockey β high probability of impact, can directly cause kidney trauma
- Combat sports: Boxing, taekwondo, sanda, MMA β direct strikes to the flank carry extremely high risk
- High fall risk: Horseback riding, rock climbing, skiing, skateboarding β falls may cause severe flank impact
- High intra-abdominal pressure: Weightlifting, deadlifts, heavy squats β sharp increases in intra-abdominal pressure can compress the kidneys
Relative Contraindications (Require Medical Assessment)
The following exercises are not absolutely prohibited but require a doctor's assessment based on kidney size, bleeding history, etc.:
- Running: Those with greatly enlarged kidneys should avoid (repeated jarring while running may increase bleeding risk when kidneys are enlarged); those with near-normal kidney volume may consider light jogging
- Jump rope: Large intra-abdominal pressure fluctuations and landing impact are not negligible; requires medical assessment
- Sit-ups: Significantly increases intra-abdominal pressure; planks (short duration, no breath holding) are recommended as an alternative
Cyst Bleeding Prevention
Cyst bleeding is one of the main exercise-related risks for ADPKD patients, presenting as sudden flank or abdominal pain and gross hematuria. Prevention key points:
- Avoid direct flank impact: Stay away from contact and high fall-risk sports
- Wear protective gear: If participating in activities with some risk (such as cycling), wear a flank protective pad; requires medical advice
- Those with bleeding history: Limit to the lowest-risk exercises such as walking and tai chi; other exercises require individual medical assessment
β Cyst Bleeding Warning Signs
If you experience sudden severe flank or abdominal pain, gross hematuria (urine appears like wash-water or bright red), or fever during or after exercise β this may indicate cyst bleeding or infection. Stop exercising immediately and seek medical care or call your local emergency number.
Daily Activity Recommendations
Beyond formal exercise, daily activities also affect disease management. Below are recommendations for common daily activities; specific situations require medical assessment.
- Climbing stairs: G1βG3 stage can do normally; G4 stage and above should avoid climbing more than 3 consecutive floors β use elevators to break up the climb
- Housework: Normal daily housework (sweeping, mopping, cooking) is fine, but avoid lifting heavy objects (> 10kg) to prevent sharp increases in intra-abdominal pressure
- Sexual activity: Can be normal; will not directly cause cyst bleeding. If you experience flank or abdominal pain, significant blood pressure increase, or other discomfort, pause and consult your doctor
- Travel: Travel is fine; pay attention to hydration and avoid prolonged immobility (deep vein thrombosis risk on long flights or drives); carry antihypertensive medications and medical summary; late-stage patients need medical assessment before long-distance travel
Pre- and Post-Exercise Precautions
Proper pre-, intra-, and post-exercise management can significantly reduce exercise-related risks. The following points apply to all ADPKD patients.
Before Exercise
- Warm-up: 5β10 minutes of low-intensity activity (slow walking, joint circles) to gradually raise heart rate
- Measure blood pressure: If resting blood pressure > 160/100 mmHg, postpone exercise and consult your doctor; those with unstable blood pressure should measure before every session
- Assess physical condition: If you have fever, significant fatigue, or flank/abdominal discomfort, suspend exercise
During Exercise
- Hydrate promptly: Those with normal kidney function can drink water during exercise as needed; those on fluid restriction should follow their doctor's hydration plan
- Stop immediately if: You experience flank or abdominal pain, dizziness, palpitations, chest tightness, blurred vision, or unusual shortness of breath β stop exercising and rest, and seek medical care if necessary
- Control intensity: Monitor in real time with the talk test and RPE, staying at moderate intensity (RPE 4β6)
After Exercise
- Cool-down: 5 minutes of low-intensity activity (slow walking, stretching) to gradually lower heart rate
- Keep an exercise diary: Record exercise type, duration, intensity, blood pressure, and physical sensations (see template below)
- Watch for delayed reactions: If flank pain, hematuria, or significant fatigue appears within hours after exercise, record it and consult your doctor
Emergency Signals
β Seek Immediate Medical Care
If you experience any of the following, stop exercising immediately and seek medical care or call your local emergency number: severe flank or abdominal pain (possible cyst bleeding), gross hematuria, chest tightness or chest pain (possible cardiovascular event), severe dizziness or fainting, or persistent irregular heartbeat.
Exercise Diary Template
Keeping an exercise diary helps you understand your body's response to exercise and makes it easier for your doctor to adjust your plan. Recommended items to record β can use a paper notebook or phone memo app.
- Date: Year/Month/Day
- Exercise type: e.g., brisk walking, swimming, resistance bands
- Duration: e.g., 30 minutes
- Intensity (RPE 1β10): e.g., 5
- Pre-exercise blood pressure: e.g., 135/85 mmHg
- Post-exercise blood pressure: e.g., 140/88 mmHg
- Physical sensations: e.g., "completed easily, no discomfort" or "slight flank soreness, relieved after rest"
