Polycystic Liver Disease (PLD)
Approximately 70-80% of ADPKD patients also have liver cysts. Liver cysts typically do not affect liver function, but large cysts can cause abdominal bloating, pain, and early satiety.

- Treatment β Most require no treatment; symptomatic cysts may need aspiration/sclerotherapy or surgery
- Note β More common in women, possibly related to estrogen
Hypertension
Hypertension is the most common cardiovascular complication of ADPKD, affecting 50-70% of patients. It can accelerate kidney function decline and is a key treatment target.
- Target β < 130/80 mmHg (KDIGO 2025, individualized)
- Common medications β ACEI or ARB (physician determines suitability)
- Lifestyle β Low-sodium diet, moderate exercise, weight control
- Monitoring β Home blood pressure monitoring, track trends

Urinary Tract Infection
ADPKD patients have increased risk of UTI, with incidence of 30-50%. Cyst infection can cause fever and flank pain, requiring prompt medical attention.
- Symptoms β Frequent urination, urgency, dysuria, fever, flank pain
- Treatment β Antibiotics; cyst infections require longer courses
- Prevention β Adequate hydration, don't hold urine, personal hygiene
Other Possible Associations
- Kidney stones β Incidence approximately 20%
- Cardiac valve abnormalities β Mitral valve prolapse, etc.
- Abdominal wall hernias β Slightly higher than general population
- Colonic diverticulosis β Possibly increased
- Intracranial aneurysms β Screening recommended for high-risk patients
Not every patient will develop these associations. Regular check-ups and communication with your doctor are key.
